You can build forms; configure everything from notification emails to approval workflows, to form security; and view, edit, export or manipulate the data which has been submitted via your form.

- Form Builder - Select different types of fields and menus to design your form
- Form Settings - Configure everything from notification emails to approval workflows and form security
- Submissions Database - View, edit, export or manipulate the data which has been submitted via your form
As a reminder, if you currently have patient data in Formstack, be sure you to export the data to a secure location and delete all protected data from Formstack. As of January 2021, all forms that collect protected information will be required to reside in Qualtrics.
Other software for forms and surveys is Microsoft Forms which works with Microsoft Teams.
As of June 21, 2025, new vendor requirements from Formstack requires the enablement of Multi-Factor Authentication (MFA) at login. You will now need to use MFA with Duo to verify your credentials. This is to ensure that all data and information submitted to and stored within Formstack is more securely protected so that you can continue using this application for university business.
